Sometimes information from the bystander or even the patient can clarify what exactly happened in a cardiac arrest. In King County the Medical Director calls the patient or relative as part of ongoing QA. The call is about 1-2 months after discharge or death. The medical director offers to answer any questions about what happened during the resuscitation. This is an open interview and one has to be very sensitive to the circumstances. The goal is to support the family and secondarily if appropriate to clarify what happened during the event. This form is in a Word document so that local agencies can customize it to their needs.
