A comprehensive cardiac arrest registry should ideally obtain all the following pieces of information following a cardiac arrest. Clearly, depending on the outcome, selected pieces of data will not be available. Autopsy reports are generally not performed for expected deaths. Some of this information can only be obtained as part of a formal quality improvement program.

EMT incident report
EMT AED digital recording and resuscitation summary
Paramedic incident report
Paramedic digital recording and resuscitation summary
PAD digital recording if a PAD AED was used
Hospital discharge record
Death certificate
Autopsy report
Interview with patient or spouse or partner or bystanders (in the event of an arrest in public)
